Form does not appear on Zapier

Hello,

I am trying to connect a form on Zapier but it doesn’t show up when selecting the different form names available.

Does anybody know what I am possibly doing wrong ? My form is located on the second page of my website.

You have to submit the form once for it to show up in zapier.
