Forms not working, all i get is error messages

hi all

im trying to link my forms to mail chimp but every time i submit form it is spitting out an error??? anyone else have this problem?? i have followed the instructions on webflow uni but nothing is working.

I believe the field name property needs to align between MailChimp and Webflow.
