Thanks @SidneyOttelohe. Tried doing this but the results in the first two forms are not captured when the user submits the form. Any thoughts?

Hey @Shufai. The slider element is positioned within the form element so normally everything should be captured, unless I’m missing something. :stuck_out_tongue:
