Unable to view second form in dashboard

Hi Webflow,

I am unable to view the second form I just added to our webflow site. I even rebuilt it from scratch instead of copying and pasting the form on another page. When I view the csv in form dashboard, all I see is the data entry field for our first form. When I scroll down to see our test submission forms it displays the same data entry fields as the first form, and only one field is filled.

I was able to figure it out. I did not have the text fields named differently in the forms.
